Registered Nurse Case Manager, Maternity and Pediatrics

We are seeking an experienced, compassionate, and highly skilled Maternal Health & Pediatric Case Manager to support patients across the perinatal and pediatric continuum. In this hybrid role, you will manage and oversee comprehensive patient assessments, care planning, implementation, monitoring, and evaluation to promote wellness and autonomy. Through advocacy, education, effective communication, and coordination of community and clinical resources, you will facilitate individualized care and support meaningful health outcomes, while balancing onsite collaboration with the flexibility of remote work for focused outreach and followup. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. Maternal Health Support Engage patients early in pregnancy to assess risks, needs, and barriers to care Provide education, care coordination, and support through delivery and postpartum Facilitate timely followup appointments, including TCM and specialty care Support technology readiness for virtual care and remote monitoring programs Address SDoH needs through resource navigation and community partnerships Pediatric Case Management Support newborns and pediatric patients transitioning from hospital to home Coordinate care with pediatricians, specialists, and community agencies Provide familycentered education and advocacy Monitor clinical and social needs to prevent avoidable ED visits and readmissions Population Health & Collaboration Partner closely with OB, pediatrics, nursing, social work, and community health teams Participate in interdisciplinary huddles, case reviews, and quality initiatives Contribute to program development in a growing, innovative academic health system Use datadriven tools and workflows to support highreliability care Conduct comprehensive assessments and implement case management interventions to improve maternal and pediatric health outcomes Demonstrate accountability and initiative through consistent performance of assigned caseload and responsibilities Ensure compliance with all state and federal regulatory requirements Other duties as assigned
Work Experience
Education and Experience - Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required; BSN preferred.
- 3 + years of case management experience in maternal health, pediatrics, or population health
- Experience with educating patients and patient care plan goal setting (essential)
- Case Management Certification (preferred or willingness to obtain)
Knowledge, Skills and Abilities - Strong clinical judgment with the ability to work independently and manage priorities in a hybrid work environment
- Experience in a managed care or care management environment; familiarity with NCQA accreditation standards preferred
- Comprehensive knowledge of case management, including assessment, care planning, implementation, and evaluation of patient needs
- Strong clinical knowledge of healthcare delivery systems, including utilization management, insurance benefit structures, and standards of medical practice
- Proficient criticalthinking, analytical, organizational, and problemsolving skills to support effective case management and efficient workflows
- Exceptional interpersonal, verbal, and written communication skills, with the ability to educate and engage patients, families, providers, and leadership
- Demonstrated commitment to patientcentered care, health equity, cultural humility, and traumainformed practice
- Proficiency in documentation and maintenance of accurate, confidential client records
- Comfort with technology, including virtual care platforms, EMRs (e.g., Epic), and Microsoft Office applications (Outlook, Word, Excel, PowerPoint)
- Knowledge of applicable state and federal laws, regulations, and community resources related to healthcare and case management
